Resuming Your GLP-1 Medication After a Break

Life happens — whether it was a shipping delay, a vacation, or you simply needed a pause, missing doses of your GLP-1 medication (Semaglutide or Tirzepatide) is more common than you'd think. The important thing is restarting safely.

Your body adjusts to GLP-1 medications over time, and jumping back in at your previous dose after an extended break can cause unnecessary side effects like nausea, vomiting, and GI discomfort. That's why our providers follow a standardized lapse-in-treatment protocol based on how long you've been off your medication and what dose you were on when you stopped.


How Your Restart Dose Is Determined

Your provider will evaluate two factors:

1. How long you've been off your medication 2. What dose level you were on when you stopped


Off 1–4 Weeks

You'll resume at the same dose you were on before the break. A short gap like this doesn't significantly impact your body's tolerance, so no dose adjustment is needed.


Off 5–8 Weeks

You'll resume at one dose level lower than where you left off. For example, if you were on dose 4, you'll restart at dose 3. This gives your body time to re-acclimate and minimizes the chance of side effects.


Off 8–12 Weeks (Previously on Dose 4, 5, or 6)

Because you were on a higher dose and have been off for a significant period, your provider will restart you at dose 2. Jumping back to a higher dose after this length of time increases the risk of strong GI side effects.


Off 8–12 Weeks (Previously on Dose 2 or 3)

Since you were on a lower dose, your provider will restart you at dose 1 — the introductory dose — to safely rebuild your tolerance.


Off More Than 12 Weeks

Regardless of where you left off, you'll restart at dose 1. After three or more months without medication, your body needs to re-titrate from the beginning just like when you first started treatment.


What You Need to Do

If you've had a lapse in treatment and are ready to restart:

  1. Click "Chat" (left menu on desktop, bottom menu on mobile)

  2. Click "Doctor" to message your provider

  3. Let them know how long you've been off and what dose you were last on

  4. Your provider will determine the appropriate restart dose and send your prescription to the pharmacy

Do not self-adjust your dose or use leftover medication at your previous dose level without provider approval. The restart protocol exists to keep you safe.


Why Can't I Just Restart Where I Left Off?

GLP-1 medications work by gradually building up in your system. When that buildup clears during a break, reintroducing a high dose can overwhelm your GI system and cause significant nausea, vomiting, or diarrhea. Stepping back down protects you from those side effects.


Will I Lose My Progress?

A temporary dose step-down doesn't mean you're starting over on your weight loss journey. Your provider will titrate you back up on a timeline that's appropriate for your body. Most patients return to their previous dose within a few weeks.


Does This Apply to Both Semaglutide and Tirzepatide?

Yes. This protocol applies to all GLP-1 and GLP-1/GIP medications prescribed through GoodGirlRx.


What If I Only Missed One or Two Doses?

Missing a single weekly dose within your normal treatment cycle is different from a full lapse. If you missed one injection but continued the following week, you likely don't need a dose adjustment. Message your provider if you're unsure.


Can I Request a Higher Restart Dose?

Your provider follows this protocol for your safety. Dose adjustments are a medical decision and cannot be overridden by request. Trust the process — you'll titrate back up quickly.
